USMLE Step 2 CS Study-plan for Beginners ...15 Days ?

Study ! Study ! Read ! Read ! Oh I am sure you heard it again and again that the key to success in CS is study, study, read ... Yeah Right !
I say it is confidence, confidence - which comes with practice, more practice and studying the right thing.

The first natural question before studying for any USMLE Step : "So where exactly do I begin ? " Let's try to get a plan here. For the USMLE Step 2 CS, trust me, 15 days are enough ! - But for those 15 days , study as if there is nothing else in life. Eat CS, Sleep CS and Drink CS ;-) AND - of course the regular cliche = study smart !

From my experiences, observations of my pals and from tons of CS experiences posts on various USMLE forums, 80-90 % cases on the actual exam are those that are already covered in First Aid or USMLEworld. The specific cases per se may be modified - but scenarios pretty much stay the standard, which is good for us.

So people, when you flag those 15 days for CS study , have a war plan:
1. Study Material For Step 2 CS: Have the notes ready. As the polls on this blog suggest, UsmleWorld seems to be the leading study material - no I am not a USMLEWORLD advertiser - I used FirUSMLE Step 2 CS Tipsst Aid. The most ordered book by visitors on this site seems to be Used CS Portable Review book available for under 11-12$ - and another hot favorite Step 2 CS for the IMG under 29.99 $- Not bad :-). But of course, it is not a bad idea to just stick to good old time-tested First Aid for Step 2 CS . The 2007 First Aid Edition is now the latest edition on the market - so when you buy it through these links, you know what edition to look for :-)

2. The first 4 days - start with mastering D/Ds and Data-Collection for each of the cases on FA / UW - Use the mnemonics scattered all over this site ! Compile them....

3.Get yourself a 'clinical guineapig' ready for practicing history taking & clinical examination - preferably another person preparing for the exam, but your friend, girlfriend, boyfriend is fine too. On that note - would it not be great to get in touch with a person taking an exam at the same center and the same date ? You could hope to find one at USMLEbuddies.com. Do not forget to enlist yourself on the list, so that someone else looking could locate you too :-)

4. Know the enemy ! Familiarize yourself with the exam pattern & what is expected of you. Read about how the CS exam is graded and scored Here. Remember that, generally for each encounter you are expected to: 1. take relevant history, 2. do the physical exam, 3. explain your differential diagnoses to the patient along with the work-up plan , 4. counsel the patient and 5. answer any doubts / apprehension / challenge questions that the patient may have - all in 15 minutes.

5. Next, go through the simple & logical history-taking steps I have compiled at "Art of History taking" , on this blog.

6. Then Practice cases with your partner! The goal of practice is to develop your own comfortable history taking & physical examination style and to keep practicing it to make a standard dialogues protocol blueprint for yourself. As a guide, I have posted my generic dialogues protocol for CS encounters - including closing statements at this link - "Standard SP Encounter Protocol", which can also be accessed from the links on the sidebar of this site. In fact, that's how this blog derives it name CSprotocol ;-) Once you prepare that - keep revising the dialogues within your head whenever you get spare time - like when you are bathing, travelling, cycling, etc. I personally think that doing this was one of the most vital things that boosted my confidence, because once I had the pattern on the tip of my tongue, I knew that those 15 minutes we get for each patient were going to be enough and very comfortable !

7. And ofcourse, besides your primary reference material, read all sections on this blog for tips, tricks and fun mnemonics for cases. Also do have a look at the Free Clinical Examination videos for step 2cs - the links are present on the Left-Side bar of this blog.

8. Under No circumstances will you take the Patient Note (PN) Lightly :-) Read about saving time and strategizing the PN Here. (Sample Patient Note Included)

9.
Finally, to be practical and economical, decide your center fast and book your hotel early, so that you get better deals. To make your life easier, Hotel Reviews for USMLE Step 2 CS sorted by exam-center are compiled at Stay4CS.blogspot.com.

Soooo to reiterate ...

15 days are sufficient to study (8-10 hours a day of practice)!

I used the reverse technique to prepare for the exam - started with memorizing the D/Ds for each of cases in FA and practiced writing Patient Notes for these within the stipulated 10 mins. After I had a good command over D/Ds, I moved on to history taking and examination with a clinical study partner.

It takes anywhere between 8-10 weeks for the results to come ! In the later half of the year, it may even take more than 12 weeks.

About safety at home:

"Its important for the family to get involved in offering a structured home environment for the patients safety. Some important points:
1. Patient's belongings must be always be placed at a fixed, easily reachable place
2. All Furniture around the patient's room and surroundings must be placed in a way that will not hamper or endanger the patients movements in the house
3. Slip-resistant mats for restroom and bathroom
4. Move the patient to a room which does not need stairs to access
5. Be on the alert to keep checking on patient-falls

I would not recommend those unless you have been away from clinical practice too long, not sure of confronting american patients , not sure of english or jst too low on confidence...,

First Aid read twice and practise was all that I felt was necessary.So I did that and had a very comfortable and easy exam which I cleared.
But from what have heard from friends and colleagues ,UW seems equally good but a more expensive option.
Also in reference to a question asked here previously I too missed washing hands in my 1st two cases :)).

dear digi doc,
I thank you a lot for your advice and help.. it helped me a great deal b4 the day of the exam.
I am not much of a mnemonics person but i found out i needed them all during the exam.
A point i thought you should add somewhere in ur blog-:
While practicing with a patner make sure that ur answers are timed like real patients. I didnt finish any of my patients in time.
Am actually afraid that I might fail because there were imp questions remaining and some physical exam too.
If you can get to practice H&P on diff ppl and are able to get the rhythm I think one is ready to give the exam. Practicing with diff ppl gets rid of the nervousness.
All the best to exam goers.

Hello - things go wrong now and then in life, the sadness will phase out slowly in its pace :-), meanwhile see if retaking the CS will get you results on time [ before Jan end] and if dates are available. If yes,. then retake it - read the data-collection points better and make mnemonics for remembering stuff well. Research papers, etc will help you get interviews, do apply on time

Yes many programs do, and that also tells you why its important to call up programs before hand to find out if they will look at your application or not, before applying.

From the 'status' drop down in the exam section - choose "will take" and mention the month. Do so when you have take a new date for the Step 2 CS. It wont be lying since you will be taking the test again..and the transcript will have the 1st fail status mentioned anyways

I have not used it myself - First AId was good enough for me - but a review on Amazon.com about the book by a user without any commercial interests was positive. Do not give more than 15-20 days for the prep, but keep those days for intense practice and reading..u can always buy the book from amazon and sell it back for almost similar price once u are done..g/l

Ya , i got a date on feb 6th 2008 earlier, then i put in dates suitable for me and gave them my yahoo and gmail id for each request, we can select a range of 30days for each id given, and kept my gtalk and yahoo messenger open throughout, enabling the play sound when recieving message on yahoo messngr, so whenever i got a mail i was notified and atlast i got my desired date in 3-4 days

Wow - great site!! I have 2 questions: (1) what resource did you use to master your D/D and data collection? My exam is in 2 weeks and I am having a real problelm with this area. Without a good D/D, it affects my data collection and PN because I lose so much time.

(2) How do I master a 10-minute PN? I can not ocmplete the PN in less than 12 minutes in my practice sessions even though I have tried several styles of writing it. The faster I write, the worse my handwriting becomes. Please help :(

Thanks for your comment - I mostly used First Aid, a little bit of Wrongdiagnosis.com and plus medical school knowledge/step 1 prep knowledge. Eventually note that on the exam, you need to list out the D/Ds that apply to your case in the decrease order of relevance.

Broken sentences are fine instead of elaborate long ones. Check out the second style of Patient note mentioned on Patient note section of USMLE.org Here

There are no centers where there are no failures...this time you say Philly,sometime back some advised against Los Angeles and some even said Chicago, Houston and Atlanta :-)

So, as mysterious and non-transparent the scoring system is already - center rumors always come from the negative experiences. Those cannot be trusted - what can be trusted is your ability to practice, study well and be alert about study tips and trends thru this blog, forums..and yes - a very important priority is to take the test early :-)

The only reason I would change a center is if i were getting a earlier and more convenient date
